Welcome to the SURREY HELP FILE This information file is designed to function as a directory to services for children with disabilities. In the SURREY HELP FILE we have tried to cover a broad range of topics. It is intended as a first source that will give the basics and provide pointers to where to find out anything else that you want to know. Where organisations are given, we have tried to provide not just addresses but a briefing on what they offer. For those who want to learn more about any topics, we recommend you contact the service or department direct. The SURREY HELP FILE is organised into sections containing their own factsheets. Within each factsheet the headings are usually in alphabetical order. Topics often overlap and useful information may appear in more than one place. The final section contains a comprehensive address list of charities and organisations that may be able to offer you more help. We have tried to keep abreast of changes, but inevitably information will quickly 'date'. It does not recommend services nor guarantee the quality. You must always discuss your needs with the service provider. Primary Health Care Team The Primary Health Care Team consists of General Practitioners, Health Visitors, Community Nurses and Community Midwives. The GP is responsible for the health care of all his patients and will refer a patient for whatever specialist help may be required. Health Visitors Health Visitors are qualified nurses with additional postgraduate training, equipping them with the knowledge and expertise to work with families and groups in the Community. They are based either in GP Surgeries, Health Clinics or Health Centres. Health Visitors offer support and guidance in child rearing and parenting, particularly for infants and children under 5 years. They also support parents with children with disabilities, providing advice on care and facilitating any aids, benefits and support services that may be required. Health Visitors do not work in isolation but work closely with GPs and other members of the Primary Health Care Team as well as working closely and in collaboration with other agencies, thus being able to offer and access a wide range of services. Health Visitors also work closely with School Nurses in supporting the care of children with special needs in the school setting. All children are invited to attend Child Health promotion and developmental assessment clinics provided by their GP, HV or Community Medical Officer. If there is any cause for anxiety, a referral is made by a GP for further investigation by a Consultant Paediatrician. Mother’s are invited to bring their children for protection against diphtheria, tetanus, pertussis (whooping cough), polio and haemophilus influenza B (HIB) at 3 months for a course of 3 monthly injections. Vaccination to protect children against measles, mumps and rubella (German Measles) is offered at 12-15 months. Your Health Visitor will have information about local services. Community Midwife Specialises in preparation for pregnancy, assistance during pregnancy to maintain good health and to prepare for childbirth. The Midwife provides follow up care for the mother and baby both to the Maternity Unit and at home for the first 10 days before handing over care to the Health Visitor. HOSPITAL AND COMMUNITY SERVICES Most districts have a Child Development Centre that may be based in a hospital or in the Community. A child may be referred to a centre for an assessment by a team of specialists which will include a Consultant Paediatrician, a paediatric Speech and Language Therapist, a paediatric Physiotherapist, an Occupational Therapist and a Clinical Psychologist. A Social Worker or a representative for the local Education Authority may also be available. The child’s general health, hearing, eyesight, communication skills, mobility and social and emotional development will be assessed. All parental concerns are addressed and joint decisions are made between the parents, medical and therapy staff about any future medical treatments and therapies. There may be a need to refer a child to other specialists or for further investigations. Clinical Child Psychologist The child clinical psychologist is a specialist in children's emotional, intellectual and behavioural development. They provide assessment and counselling and also devise programmes for parents to follow to help children develop skills. Advice can also be given on managing any sleeping disturbances, tantrums, or unco-operative behaviour. Occupational Therapist Occupational therapists work with children with a variety of conditions including cerebral palsy, developmental delay and developmental co-ordination disorders. The O.T. can assist your child to develop personal skills by devising individual programmes, and to develop their fine motor skills. They can also advise you about mobility and seating. Therapy is provided at centres, in mainstream and special schools or in a child's home. Paediatric Physiotherapist A Paediatric Physiotherapist will advise parents on positioning, handling and the use of exercises to help prevent the development of muscle contractures when muscle tone is impaired, and to assist a child whose motor development is delayed. The family and therapist work together incorporating treatment into daily life. Speech and Language Therapist Assesses a child’s development of communication skills and continues to offer therapy and advice as long as it is required. Therapy may take place in the home, in a child development nursery or in a school environment. Programmes of therapy are carried out by all involved in the care of a child e.g. parents and family members, playgroup helpers and teachers. Special Care Baby Unit Some babies require very special nursing and medical care following delivery because they are premature or have problems requiring very close observation or specialist nursing and therapies. Your GP and HV will be kept informed. If your baby is receiving care in a Special Care Baby Unit it is likely that he or she will be followed up after discharge in a paediatric clinic. Children's In-Patient Care If admission to hospital is necessary at any time your child will be cared for on the children's ward by Paediatric Nurses and Paediatricians. The ward staff will encourage you to spend as much time as possible with your child during the stay. Facilities for parents are available in most hospitals. If your child has nursing needs after discharge a specially trained Paediatric Nurse may visit you at home to carry out any nursing needs. Paediatric Out Patients Hospital based Paediatricians provide outpatient clinics for treatment and follow up of children’s medical problems and following hospital admissions. Hospital Social Worker The Social Worker is pleased to meet parents who have children with disabilities while in hospital and will liaise with the Specialist Disability Team for your home district following discharge. Together you can discuss immediate and long-term worries and concerns and assess your needs in order to identify the help, support and services necessary to assist you in caring for your child. The Social Worker can put you in touch with other parents through the Face to Face befriending service, (see Section 3 factsheet 4) parent support groups and counselling services. Advice on welfare rights is also available. Genetic Counselling Parents who have a child with a disability are concerned about the risk of future children having the same conditions. The Regional Genetic Service provides diagnosis and counselling for families with inherited conditions. Your GP or Consultant Paediatrician can make a referral. Nutrition & Dietetic Service Dietetic advice is indicated in the management of many conditions. The dietician sees children who require modified diets as part of their treatment so that a diet can be devised to meet a child’s specific needs. Your GP or consultant can refer to your local service. Department of Special Care Dentistry Offers a full range of dental care to children, who due to a special need are unable to be treated by their family dentist. Patients can be referred to the service by their dentist, GP, health visitor or other health professional. An initial assessment can take place in a dental clinic or at Frimley Park Hospital. COMMUNITY SERVICES Audiology (Hearing) Within the next few years it is proposed that all babies will have a neonatal hearing-screening test. Babies who have a family history of deafness or who have had treatment or medical problems in the new-born period associated with hearing loss already have this test. At present all babies are tested at 7-9 months by Health Visitor’s and this will gradually be phased out. If there is concern about the result of a hearing test, the baby is referred to an Audiology Clinic. Advice and support will be given to parents of children found to be hearing impaired. Regular follow up assessments of hearing and speech and language development are arranged. If necessary children’s hearing aids are prescribed and can be fitted. Progress will be monitored and support given in the home by a Teacher from the Sensory Support Services (Hearing). Chiropody/Podiatry (Foot Health) Following a referral from a GP, health professional or parent/guardian, all children are fully assessed as to their foot health needs by a State Registered Chiropodist/Podiatrist. Please enquire at your local Health Centre for details of your nearest N.H.S. Chiropody Clinic. Continence Advisory Service To provide an advisory and educational service on all aspects of managing incontinence in the home. This service is for both children and adults. Your Health Visitor can put you in touch with this service. Community Dental Service Welcomes children with disability and offer's treatment and preventative advice on diet, tooth brushing and fluoride supplement. The service is normally based at your local Child Health Clinic or you may wish to use your family dentist. Dental services may sometimes be available through your child's school. Further advice on this service can be obtained from your Health Visitor or School Nurse Community Learning Disability Services This service is provided by trained nurses who have specialised in Learning Disability and provides home visiting to parents and carers of people with learning disabilities. They can offer help that can be personal or practical, counselling and advice on behaviour. Occupational Therapy Staff work with children with a variety of conditions including cerebral palsy, developmental delay and developmental co-ordination disorders. Eyes and Vision If there is any parental concern, or family history of eyesight problems, or a learning difficulty, an assessment of a child’s vision can be made by referral to a Consultant Ophthalmologist and/or Orthoptist. If a child has visual difficulties, a Teacher from the Sensory Support Services (Vision) will visit you at home and/or your child’s school to instruct and advise on techniques which assist and enhance your child’s learning skills. Child Psychiatry A Consultant Child Psychiatrist is a doctor who has specialised in the branch of medicine concerned with the study and treatment of the mental ill health, emotional and behavioural disorders of childhood and adolescence. Child psychiatrists work within multi-disciplinary teams, (see Child and Family consultation Service, factsheet 4). Community Physiotherapy Service The service provides physiotherapy assessment and treatment for children. The Community Physiotherapist will compile an individual exercise programme for your child to stimulate movement, increase co-ordination and strengthen muscle power to prevent deformity and to provide equipment to enhance mobility. Children are seen at home, in special schools, mainstream schools and nursery schools. An assessment includes help and advice to parents and carers. School Health Services The Community Child Health Department provides a service to all state schools and for children with special educational needs. The Community Paediatricians will see pre-school and school children for medical assessment in order to provide advice for the Assessment of Special Educational needs. They will also be involved in regular reviews of educational special needs and be able to offer advice to parents, schools and other professionals at other times. Each school has a named School Nurse who is available to ensure a child’s health needs are met at school. Speech & Language Services The service aims to provide an assessment of communication needs, appropriate advice and intervention as part of a multidisciplinary team. There is an early home visiting service for children with learning disabilities to assist with disorders of speech, language, fluency, sound production and feeding. Wheelchair Service Referrals to this service can be made by your physiotherapist, OT or GP for the provision of certain pushchairs, buggies and wheelchairs. INTER-AGENCY & VOLUNTARY ORGANISATIONS Child and Family Consultation Service The Service offers a diagnostic assessment and treatment service to children and their families. The majority of children referred to the service have emotional and behavioural problems. The team consists of:- Consultants in Child and Adolescent Psychiatry, Child Psychologists, Psychiatric Social Workers, who are specialists in Family Therapy and Community Psychiatric Nurses and a Principal Clinical Child Psychologist. For the address of your local service (see Section 2 factsheets 5). Deaf Services Team Work with both children and adults who are deaf or hard of hearing. This involves a range of advice, practical support and equipment service. Surrey Association for Visual Impairment maintains the Register of Visually Impaired Children on behalf of Surrey County Council. They are happy to assist you to claim benefits (such as DLA). They can also provide children with mobility training - (teaching your child to move comfortably in their environment by using senses other than vision). For further information contact:- S.A.V.I. Special Care Baby Unit Some babies are cared for in the Special Care Baby Unit because they are premature or have particular problems requiring extra skilled nursing. A Sister affiliated with the unit may visit you at home after your baby is discharged, to offer you support and advice. She will liaise with your Health Visitor. If your baby is receiving care in the Special Care Baby Unit he or she may be followed up after discharge at a Paediatric Clinic. B I S C S - Babies in Special care Support BISCS - is a group of parents who share the experience of having a baby in special care and can offer new parents support. Children's In-Patient Care If your child needs to be admitted to hospital at any time and is aged under 14 years Paediatric Nurses and Paediatricians will care for him or her on the children’s ward. Older children are also cared for whenever possible on the children’s ward. Child Development Centre The Child Development Centre brings together a team of specialists who advise parents and assess and treat children whose development is giving cause for concern. A booklet about the centre will be sent to parents with their child's first appointment or may be obtained from the Children's Unit. Children together with their parents may need to see some, or all, of the people who work on the unit. Other Services provided at the Centre: Opportunity Playgroup This playgroup for children with special needs is held in the Children's Unit on two mornings each week during school terms. Experienced volunteers staff it. The unit houses specifically directed therapy groups run by senior paediatric therapists and supported by nursery nurses. A fast track multidisciplinary assessment may be offered to specific children aged three and over, to establish their needs and formulate a treatment plan-incorporating liaison with external agencies. Department of Child and Family Psychological Medicine These specialised units are based in St Peter's Hospital and Ashford Hospital, it's team consists of Consultant Child Psychiatrists, Clinical Child Psychologists Psychiatric Social Workers and Clinical Nurse Specialists. The team work with individuals and families and offer advice and treatment to children who have severe sleep disturbances, food fads/disorders, behaviour disorders, self-harming tendencies, and other related problems. Referrals are made either through your GP, Paediatrician or Social Worker. Child Assessment Service The Service is based at White Lodge Children's Centre, Holloway Hill, Chertsey. It provides a multidisciplinary and multiagency assessment for children under five with complex needs and disabilities. Professionals from within Woking Area Primary Care Trust and Surrey Children’s Service (Education and Social Care) work together to provide a co-ordinated assessment report that will include a comprehensive and accurate profile of your child's strengths and needs. This will enable 'The Team' to draw up an individual plan for your child, integrating care, therapy and education. Assessment may take place either at the centre or at home. Special nursery sessions can be set up at the centre if you feel your child is more at ease in this environment. Parents will be invited to participate fully in assessment sessions. These can range from one session to several sessions over a six-week period, concluding with a written report and meeting to determine your child's future needs. A member of the team will be identified to co-ordinate and monitor the agreed plan. Health or Children’s Services can make referrals. An information leaflet about the service will be sent to parents with their child's first appointment. Other Services provided at the Centre Toy Library - White Lodge Centre This well stocked Toy Library lends toys to any child with a disability, not just those who are members of White Lodge. All the toys are suitable for young children with disabilities and are available on payment of a nominal fee. If parents are looking for a particular toy recommended by a therapist, the library would aim to get it. Maple Children's Centre 28-34 Wolverton Avenue Kingston upon Thames KT2 7QB Telephone: 020 8546 7711 ext. 2690 This Maple Children's Centre provides specialist services offering treatment, therapy and support for children from birth to 19 years. Some staff make home, school and nursery visits. There is also a Short Term Care Unit to which children are admitted for assessment, treatment or overnight care. Maple Playgroup This playgroup meets twice a week and is for children under 5 years of age who need specialised play and support. Summer Play Scheme The play scheme for children aged between 2 and 8 years, runs for 2 days a week during August. There are a number of special groups run by staff to which parents and children are referred: Eating Disorder Clinic Run by CAFC- Child and Adolescent Family Centre (formerly Child Guidance). Enuresis Clinic Staffed by paediatrician and school nurse. Movement and Music P.A.C.E. (Paediatric Constipation and Enuresis Clinic) The clinic is held once a week at Roselands Clinic, New Malden, includes- paediatrician, nurse, psychologist and dietician. Pre-School Communication Group Speech and language therapy and portage. Swimming Group Held at Bedelsford School Respite Care Maple Children's Centre Short Term Care Unit can also provide overnight care to children with health needs. SWAPS Support group for parents of children with Autism and Aspergers Syndrome, meets second Tuesday every month at 8pm. Discussions, speakers, videos - also social events organised for children and their families.

Children with their parents can meet a variety of professionals for treatment, therapy and advice. The group gives parents the chance to chat over coffee. During the morning children are seen individually or in groups by one or more professionals. There is also opportunity to play. In addition parents occasionally arrange social activities, amongst themselves, such as outings or meals. Contact information: Ward Manager Telephone: 01737 768511 ext. 2610 / 2611 Special Care Baby Unit Newborn babies who need extra skilled nursing and medical attention for any reason are cared for in SCBU which is situated near to the Delivery Suite and Post natal wards. There are close links with Regional Neonatal units where some babies may be transferred for further Neonatal Intensive Care. The nursing staff will liaise with your own health visitor on your baby’s discharge for support at home. If your baby has continuing nursing needs at the time of discharge, it is likely that a member of the Community Nursing Team also will be involved. Children's Outpatient Clinics There is a purpose built children’s outpatient department at East Surrey Hospital. A team of paediatricians, including the paediatric consultants holds daily general paediatric outpatient clinics. There are specialist paediatric clinics such as Asthma, Diabetic, Neonatal and Seizure clinics conducted by the consultants. Regular joint clinics are held with visiting specialists from regional centres in paediatric Cardiology, Neurology, Surgery, Endocrinology. Satellite consultant clinics are being held at Caterham Dean, Oxted and Dorking Hospitals. The route of referral for above clinics is mainly through your GP. The legislation says that, where a child is assessed as having a disability and therefore by definition is a child in need, the local authority must provide services appropriate to that child's needs in order: · to safeguard and promote the child's welfare; and,· as far as is consistent with that duty, to promote their upbringing by their family.Eligibility Criteria Many of the duties imposed on the Local Authority in respect of children are general duties. The use of Eligibility Criteria is a means of ensuring equitable access to services whilst at the same time complying with the statutory duties in respect of children. It is also a means of allocating resources to the areas of highest need. Our decision as to the delivery of a service will therefore be based upon the outcomes we believe we can achieve for children. An initial assessment of children in need will be undertaken within the time scales specified which identifies the child’s developmental needs and takes into account the capacity of the parents to meet these needs. Family and environmental factors such as family functioning, housing and employment will also be assessed. How Do I Contact Children’s Service Social Care? Your first point of contact with Children’s Service Social Care should be the Assessment Team for your area. The worker there will discuss your requirements and if appropriate refer you to the Children with Disabilities Team. Teams for Children with Disabilities and Their Families Team Manager (East) Team Manager (West) The two teams (East and West) were formed in April 1997 to provide and develop services dedicated to the needs of children with a registerable disability and their families. It is the responsibility of the social worker, in conjunction with other agencies to provide you with an initial assessment of need. If after assessment the child meets the eligibility criteria, the social worker will agree with the parent carer the most appropriate way of meeting the need. This may involve working with a range of statutory and voluntary organisations in your local community. Priority for services will always be given to children and families in greatest need. Some children may therefore be directed to other appropriate sources of help and support. EDUCATION SURREY'S SCHOOLS & EDUCATIONAL PROVISION Primary Education Primary education is provided to children aged 4 to 11 years and may take place in either Infant, First, Junior or Primary Schools. The vast majority of children with special educational needs, given appropriate support, are educated in primary schools. Secondary Education All of Surrey's secondary schools are comprehensive schools and cater for children of all abilities. The majority cater for the age range 11-16, from which students will normally transfer to Sixth Form College or a College of Further Education. Some schools cater for students aged 11 to 18 years and have sixth form provision as part of the school. As with primary education, a similar provision is made for children with special educational needs. Special Education The LEA has 25 special schools, which provide for children with a wide range of special educational needs. In addition there are 39 day units attached to ordinary mainstream schools. Further information can be obtained from one of the four local education offices listed below. Educational provision for children unable to attend school The service provides education for children of statutory school age who are out of school as a result of illness, exclusion or any other reason. Teaching may be at a Pupil Referral Unit, a hospital, at home or other venue and will be individually planned to suit the needs of your child. The teaching will normally be provided when your child has been out of school for 15 days. The service will support you and your child in making a successful return to school. Individual Education Plan Whenever a child has been identified as having special educational needs, his or her school will ensure that an individual educational plan is drawn up in consultation with parents. The plan will identify individual work programmes, performance targets, review dates, findings and decisions and information from other professionals such as; speech therapists, physiotherapists, etc. The plan will be reviewed regularly by the school to ensure progress. Parents should have a copy of the IEP. Annual Reviews If your child has a 'Statement' it will be reviewed annually by the LEA. In Year 9 a special type of review is held. The review is designed to be multi-agency and to ensure a smooth transition for the young person from when they reach 16 up to adulthood. For any child who is registered or registerable as disabled Children’s Service will offer a full assessment of need during their final year in full time education in accordance with the Disabled Persons Act 1986. (Contact your social worker for more information). Surrey Children’s Service will send you and your child a booklet "Options for the Future" when they are 12 to help you to plan. Further Education Further education (FE) normally refers to the period of education between the ages of 16 and 19 years. In approved cases this will include attending an independent special school or college. The Further Education and Higher Education Act can extend the age to 25 (your child must have approved funding). Your child may stay on at his or her own school or attend a local college. Many local colleges offer specific courses for pupils with special needs. The courses are generally full time and last between one and three years. Frequently a Community Service Volunteer, who can help with a range of needs, including mobility and personal care, assists children with disability at college. For further information: ask your child's school for a copy of the Careers Enterprise Booklet 'A Parent's Guide'. Grants for Further Education Surrey residents applying for, or following, full-time further or higher education courses may qualify for a grant. The type of transport used depends upon individual needs and ranges from buses to hired taxis and coaches. If necessary the Authority employ escorts to accompany children on the journey to and from school. Parents who want to know more about this should contact the Local Education Office first. If you choose to transport your child in your own car, you may be eligible for help with the cost, dependent on individual circumstances. Speak to the Headteacher of your child's school about this first. SPECIAL EDUCATIONAL NEEDS What are special educational needs? As many as one in five children may at some time need extra help with their education and they are said to have "Special Educational Needs". There are varying levels of Special Needs, including a small number of children who have severe problems in learning and who need to have special kinds of help over several years. These children are likely to have a written description of their needs and what is to be done for them. This is called a Statement of Special Educational Needs and is drawn is by the Local Education Authority (Surrey County Council). Nowadays most children with Special Educational Needs attend mainstream schools, with additional support provided where necessary. A minority of children may benefit from the gentler routine, the smaller groups, or the very specialised teaching and educational aids available in special schools and classes. There are various stages to the process of assessing your child’s needs. Pre-school children with special educational needs If you think your child is slow in developing or is not hearing or seeing properly speak to your nursery or playgroup leader, health visitor, your family doctor or someone at your local Child and Family Consultation Centre. It is important to get help as soon as possible. Children with serious learning difficulties benefit greatly from early help with their education. Surrey has a home teaching service, called Portage Early Education Support Service, which is for young children whose development is delayed. There are also visiting advisory teachers for children with serious visual or hearing difficulties from Physical and Sensory Support Service. (Section 2 Factsheet 9) These services will help you to develop your child’s skills. There may be a place for your child in a nursery school class, or extra help at a playgroup. (see Sections 3 and 7) School age children with special educational needs In Surrey, there is a clearly defined statutory assessment process which helps schools plan programmes to support children’s learning in partnership with their parents. This section guides you through the various stages of the Assessment Process and explains the criteria required for a statutory assessment. (Code of Practice, 1996 Education Act) What is the Code of Practice? The Code of Practice on the Identification and Assessment of Special Educational Needs (SEN) came into effect on 1 September 1994 and was issued by the Government to provide guidance to Local Education Authorities (LEAs) and governing bodies on their responsibilities towards all children with SEN. The 1996 Education Act stated that LEAs and governing bodies must have regard to the provision of the Code. Schools must consider what the Code of Practice says when they draw up their policies for children with special educational needs. Among the things the schools policy will tell you are: · The name of the teacher who is responsible for children with special educational needs ( often called the Special Educational Co-Ordinator (SENCO))· The school’s arrangements for deciding which children need special help and their plans for giving that help, stage by stage· How the school plan to work closely with the parentsYour child’s school must give you a copy of their policy if you ask for one.
What is the special educational needs assessment process? The first stages of the process are school-based: School Action (formerly Stage 1 and Stage 2) aims to help the child get more help from his or her teacher. If this does not overcome the child’s learning difficulties, the school will involve the child’s parents and draw up an Individual Education Plan. Then, if this then does not help overcome the child’s learning difficulties, the school will put into practice: School Action Plus (formerly Stage 3 and Stage 4) when the school involves specialist help from outside the school. If the child still does not progress, the child’s case may be considered by the Area Special Educational Needs Panel which will decide if the child’s needs are severe, so as to require a Statutory Assessment. A Statutory Assessment means that the child will be seen by an Educational Psychologist and also by a doctor or medical officer who will advise the Local Authority on the difficulties and learning needs of the child. The professional advice received during the Statutory Assessment will then be considered by the Local Education Authority which will decide if the child requires a Statement of Special Educational Needs. A Statement of Special Educational Needs is a document which sets out the child’s needs and states the special educational provision the child requires. The Statement will also determine the type and name of school at which the child’s needs can be met. Most children respond to help within the school at School Action and School Action Plus. Proposed Statement If as a result of the assessment, it is decided to make a formal Statement setting out your child's special educational needs, this will specify the special educational provision considered appropriate to meet these needs and any additional help required. You will be sent a Proposed Statement along with all the reports that have been collected during the assessment. You will be given 15 days in which to respond to the Proposed Statement. If there is any aspect that you are not certain about or are unhappy with, make an appointment to see your child's case officer at the Local Area office. You are invited to express a preference for the educational establishment you would like your child to attend. You cannot formally appeal against a Proposed Statement; this is a time for consultation and discussion between you and the Education Authority. Final Statement After considering your views, the Authority will decide to either confirm the Proposed Statement as a Final Statement or issue a revised Final Statement, or make no statement at all. You will be told, in writing, of the decision. If you disagree, you have a right to appeal and you will be advised how to do this. There is a formal duty on the Authority to review the statement every 12 months on the anniversary of its completion. Post 16 young people with special educational needs Education for young people with special educational needs does not stop at 16. Depending on your child’s interests and abilities, he or she can stay on at an ordinary or special school, or can move on to a college for further education. Many schools have developed "link" courses with colleges so that pupils in their last years at school can go to a college on a part-time basis. These courses help to prepare young people for further education by letting them experience a college and try out a range of new subjects. Further education is available for young people over the age of 16. If your child has a statement of special educational needs, further education will be considered when the Transition Plan is drawn up when he or she is 14. Many students with learning difficulties or disabilities attend ordinary courses at college with the help of special equipment or support. Some colleges run special courses or have units for these students. Your local college will have a member of staff for more information or advice. Students whose needs cannot be met by a college of further education may be able to get a place at an independent specialist college. Universities and colleges offer education at a level beyond GCSE A-level standard for students from about the age of 18. It is open to all young people who have the ability to benefit from it. If you have any questions about the choices open to your child after 16, the careers service can provide help and advice to you and your child. Help available to parents The 1988, 1993 and 1996 Education Acts and SEN and Disability Act 2001 gave greater powers and responsibilities to individual schools. The SEN Code of Practice 2001 gives guidance to everyone on how to implement the law. Parents’ involvement is very important. The 'Statementing Process' is sometimes complicated and may cause some parents to feel stressed if they think they are unsupported through the procedure. Help is available from a number of sources. Educational Services Available Before the Age of 3 Years It is generally recognised and acknowledged that parents are the main educators of young children. The first stage of learning for your child is usually achieved through creative play, social interaction, and natural expression. Stories, games, songs and specially chosen toys and materials address the needs of young children. Your Local Library has a wide selection of books, video and audiotapes that you or your child may hire. Toy Libraries are full of educational toys that you can borrow for a nominal fee; The National Association of Toy and Leisure Libraries (registered charity) publish a wide variety of books and leaflets on play. They can also advise you about your nearest toy library. EDUCATIONAL PSYCHOLOGIST Educational Psychologist The Surrey Children’s Service provides the Local Education Authority with a range of psychological services designed to enhance the learning, development and general well being of children and young persons. Educational Psychologists are trained to work in partnership with teachers and parents to investigate the educational needs of children and to advise action plans to meet those needs. Transition from children’s service to adult & community care There are many transitions in a child/young person’s life. The transition to adulthood presents all young people and their families with many challenges. However, for a young disabled people this can be more difficult due to all the different agencies involved, choices available and the planning needed. Surrey Children’s Service (Education and Social Care), Careers/Connexions, Health and Voluntary Services all work together to make the process as smooth as possible for young people and their families. Transition Protocol Surrey has a multi-agency transition protocol that details all the different actions that need to happen from the Year 9 Review onwards. We have produced a booklet called "Options for the Future" for parent carers and a similar guide for young people in an easy to understand format. Email: office.da@dial.pipex.com DISABILITY LIVING ALLOWANCE Disability Living Allowance (DLA) This is a tax-free benefit for adults or children with an illness or a disability who need help with personal care; with getting around - or both (more help or supervision than other children of the same age who do not have their particular problems). DLA is not means tested and can normally be paid on top of any other benefits you receive. Getting Disability Living Allowance: - does not normally need a medical examination. Disability Living Allowance is payable in two parts and either or both can be claimed. Care Component - If you have a child aged 3 months or over you may be able to get DLA for help with personal care or extra supervision because they are ill or disabled. For example things like washing, dressing, using the toilet. If they are 16 or over this can include preparing a cooked main meal. You can get DLA even if no one is actually providing the care needed. Mobility Component - If you have a child of 3 or over who:- cannot walk at all or has difficulty in walking because they are ill or disabled, can walk but needs someone with them to make sure they are safe or to help them find their way around most of the time. To get DLA they must normally have needed help for at least three months and be likely to need it for at least a further six months. Current Weekly Rates: - DLA provides three rates for help with personal care and two rates for help with getting around. You may also get a Christmas Bonus Revisions and appeals - if a claimant is dissatisfied with a decision on their claim they can ask a Decision Maker to look at the decision again. A Decision Maker will revise the decision on any grounds if the request for revision is received within three months of the date the decision was sent. A different Decision Maker will carry out the revision. If the claimant is dissatisfied with the decision they may appeal to an independent tribunal. You normally only have one month in which to appeal. If you are not happy with your decision, you can then appeal to a Disability Appeal Tribunal and you will be invited to meet the Tribunal and explain the disability and effects. The Tribunal consists of three people, a legally qualified Chairperson, a doctor and a lay person with experience of the needs of disabled people or someone who is themselves disabled. Hospital and Special Accommodation Effect on DLA payments Both your mobility and care component can normally only be paid up to 28 days in hospital. You must notify the DLA Unit if your stay is likely to be longer. Going Into Hospital If your child is under 16, the care and mobility components stop being paid once your child has been in hospital for a full 84 days. Going Into Residential Respite Care During periods of residential care you are still entitled to claim the care component for the first 4 weeks of care, with a 28 day linking rule. The 28-day linking rule applies if you use a respite unit on a regular basis. When care is only for short periods of time and 29 days or more separates each admission, then you may keep your full allowance. You need only notify the DLA Unit of admission and discharge dates. If you use the unit on a more regular basis the care component may be reduced or stopped. Do take further advice on this rule. FINANCE CARER’S ALLOWANCE INCOME SUPPORT INCAPACITY BENEFIT & JOBSEEKERS ALLOWANCE Carer’s Allowance (CA) This is a new name for Invalid Care Allowance. It is a weekly taxable allowance for anyone who spends at least 35 hours a week looking after someone who gets the DLA care component at the middle or higher rate. It is payable to anyone aged 16 or over. People aged 65 and over can now claim as well. You can earn up to £77pw and still get CA, but you will not be entitled if in full-time education. Any Income Support you receive will be reduced by the amount of CA you claim. However, it is still worth claiming because it will entitle you to a "carer premium" worth an extra £25.10pw. CA can be backdated up to 3 months providing that you satisfied the conditions throughout that period. How to Claim Claim form DS700 Invalid Care Allowance (also ask for FB31 Caring For Someone) from your local Social Security Office, Citizens Advice Bureau, Post Office, Hospital or Social Care Department. Current CA rates: £43.15 per week Income Support (IS) This is a benefit for people aged 16 or over whose income is below a certain level. To get Income Support you must be: incapable of work through sickness or disability; looking after someone with a disability; registered blind; work less than 16 hours a week, or if your partner works for less than 24, you may get help to top up part-time earnings or other benefits. Your right to IS does not depend on National Insurance contributions. You, or you and your partner together must not have over £8,000* in savings. Savings between £3,000 and £8,000* will affect the amount you can get. If you live permanently in a residential care home, you will not get benefit if your savings are more than £16,000. Savings between £10,000 and £16,000 will affect the amount of benefit you get. * £12,000 if you or your partner are aged 60 or over. How much you get The amount you get depends on, among other things, how old you are, whether you have a partner, whether you have any dependent children and how old they are, whether you or anyone in your family has a disability, how much money you and your partner has coming in each week. Your Income Support payment may be made up of three parts: Personal Allowances are weekly payments towards day to day living expenses for you and your partner, if you have one, and for any children or young people for whom you are responsible. Premiums are weekly additions to IS personal allowances for some people who have extra needs: 'Family Premium' - If you have at least one child. Disabled Child's Premium - If you have a child who is receiving Disability Living Allowance for personal care or mobility at any of the rates. This premium is not always paid automatically so make sure that you claim if you think you qualify. Enhance Disability Premium – If you or your child gets the highest care component of DLA. Carer's Premium - If you receive the Carer’s Allowance remember to claim. Housing Costs to help with mortgage interest and certain other housing costs not met by Housing Benefit. If you or your partner are under 60, you will only get limited help. If you get IS you, or anyone you claim for will also get free prescriptions, dental treatment, help with the cost of glasses, free school meals, help with cost of travelling to hospital and free milk and vitamins. IS is also a qualifying benefit for Social Fund Community Care Grants. How to Claim Claim form IS1 Income Support from your local Social Security Office. Claim forms for Council Tax Benefit and Housing Benefit are automatically included with your I.S. claim. Other Information It may be worthwhile you claiming IS if you are over 16 and have a disability. You are still entitled to IS even if you are still attending school and you are unlikely to work in the future because of illness or disability. You will need to produce a Doctor's sick note or letter. Incapacity Benefit (IB) for Young People IB is a tax-free benefit for people who have been unable to work for at least 28 weeks because off illness or disablement, but have not paid any or enough NI contributions. You must be aged between 16 and 20 (or 25 in some cases) when you claim. It should always be claimed by you if you are over 16 and have a disability. You are still entitled to IB even if you are still attending school and you are unlikely to work in the future because of illness or disability. You will need to produce a Doctor's sick certificate. How to claim Contact your local Social Security office for details. You can make the claim up to 6 weeks before your 16th birthday. Current IB rates: £54.40 (1st 28 weeks) £64.35 (weeks 29-52) £72.15 (weeks 53 onwards)-plus age addition £14.90 Jobseekers Allowance If you are of working age and looking after a child who does not receive the middle or higher care component of Disability Living Allowance, and wish to claim for financial assistance you may have to sign on as being eligible for work in order to receive the jobseekers allowance. If you are not required to sign on, you will continue to receive Income Support. The rules are complex, so take advice. FINANCE SOCIAL FUND, WORKING TAX CREDIT, HOUSING & COUNCIL TAX BENEFITS Social Fund The Social Fund helps people with expenses which are difficult to pay for out of regular income. Getting help from the Social Fund does not depend on your National Insurance contributions, but it is means tested. If you are on Income Support and you or someone in your family is disabled you may be able to get a Community Care Grant. This is not a loan and does not have to be repaid. For example, it could be for bedding, clothing, laundry equipment, gas and electricity reconnections, decoration, furnishings, heaters, safety items and essential travelling costs. Grants are discretionary and not for a standard amount. Your local social security office has the ultimate decision about your grant application and you may be turned down. You may also be offered a loan instead of a grant, which you would have to pay back out of your weekly benefit. For details ask your Social Security Office for Form SF300. Do take independent advice from the CAB before agreeing to a loan. Crisis Loan You do not have to be on Income Support or any other social security benefit to get a Crisis Loan. You may be able to get a Crisis Loan to help meet your immediate needs in an emergency or following a disaster where there would otherwise be a serious risk to your, or your family's, health or safety. How to apply for a Crisis Loan Contact your social security office, or the nearest one if you are away from home at the time of need. Working Tax Credit This is a regular tax-free cash benefit for low income working families with children. Working Tax Credit is for people working for 16 hours or more a week. You can claim whether your work is for an employer or self-employed. If you are responsible for at least one child aged under 16 (or 19 if in full time education up to A level or equivalent standard) you will also qualify for a Child Tax Credit. How Much You Get The amount of Tax Credit you get depends on how many children you have and their ages; the income coming into the household; any savings you or your partner may have; the number of hours you or your partner work. You may be entitled to childcare tax credit to help towards the cost of childcare. To qualify you must have arranged childcare before you send in your application for Working Tax Credit. The childcare tax credit will be 70 pence for every pound you pay out but only to cover payments of up to: · £135 for one child or· £200 for two or more children.This means the most you will get is £94.50 for one child or £140 for two or more children. Qualification for Working Tax Credit may make you eligible for other types of help such as free prescriptions, free eye tests, vouchers for glasses and fares to hospital. Housing Benefit (HB) This is a tax-free benefit paid by Local Councils to people who need help to pay their rent. You may also get Housing Benefit if you live in a hotel, guesthouse, hostel or somewhere similar and need help with your rent. You may be able to get HB whether you are employed, self-employed, unemployed or retired, and you don't have to be getting Income Support. It does not matter if you live alone or with other people, who are in work or unemployed. It is not paid to help with the costs of mortgage interest payments, fuel or service charges. How to Claim Get a claim form from your local council office (see Council Tax Benefit) if you are not claiming Income Support. If you are claiming IS you should automatically get a claim form. Council Tax Benefit The Council Tax is a local tax set by your borough or district council to help pay for local services. There is one bill per dwelling, which will usually be payable by the owner-occupier or tenant, including council tenants. If you are on low income and find it hard to pay your council tax, you may be able to get help from the Council Tax Benefit Scheme. The full Council Tax bill assumes that there are 2 or more adults in a property. If only one person lives in a property as their main home they will receive a 25% discount. You may get second Adult Rebate if someone over 18 lives with you but is on low income or Income Support. Certain groups of people will not be taken into account and will be 'disregarded' when assessing the number of adults in the dwelling. These include: Persons Caring for someone with a disability (certain criteria must be met); severely mentally impaired people. A property in which a disabled person lives may qualify for a reduction in the amount of Council Tax payable. You are eligible for such a reduction if one or more of the following apply: (i) A room which is not a bathroom, lavatory or kitchen is mainly used by and is required for meeting the needs of any disabled person resident in the dwelling. (ii) A bathroom or kitchen (which is not the only bathroom or kitchen) is used for meeting the needs...etc. (iii) Sufficient floor space to permit the use of a wheelchair required for meeting the needs ...etc. If you think a discount should apply to your home you should tell the council. Council Tax Benefit of up to 100 per cent is available for people responsible for council tax bills who are on Income Support or low income. There is no minimum contribution. The amount of benefit you can get depends on the amount of your council tax bill, your income and savings and on your personal circumstances - for example whether you have children and how many, or whether you have a disability. Social Care Minor adaptation to the home - Occupational Therapists can advise you on how to make it easier for you to manage you child at home and on what equipment and alterations to your home would suit you best. Financial assistance can sometimes be given and will involve financial assessment.
