Care Packages

Once the assessments are done the Care Manager will inform you if a care package is able to be put in place. If you and the person you care for are agreeable the Care Manager will then set to work to contact the agencies who will be supplying the services. The details of the care package will be put in writing this is called the Care Plan. This will be sent in duplicate to you and the person you are caring for. It is extremely important to read the care package document carefully. It is also important that you and the person you care for are happy with the arrangements of delivery of the care which has been put into the document. If you and the person you care for are happy with the arrangements then the person you care for should sign the Care Plan. You should then send the signed copy back to the Care Manager, making sure you retain the other one and store it in a safe place. However if you are not happy with the arrangements which have been documented in the Care Plan do not sign it and contact your Care Manager. You will then need to talk through your concerns with the Care Manager to try and resolve them.  

Care Package Reviews

During the time a care package is in place you will find that the package will be reviewed from time to time. The review which is usually done with your Care Manager will allow a review of the services helping you and the person you care for. The review usually takes the form of a meeting between you and the person you care for, the Care Manager and any other people who are involved such as the people that come in to help you. The meeting allows all concerned to voice their opinions on how they feel the care package is working and what changes, if any are needed to make it work better. Anyone can ask for a review at any time if the circumstances change
